Waiver Forms

All students participating in off-campus field trips in Integrative Biology classes should complete and sign a waiver form. Two versions of this form are provided; one for use for activities that are an integral or required part of the course, the other for activities where the students' participation is optional.

These forms should be completed to reflect the specifics of the course and activity, then distributed to students for signature. The signed forms should be submitted to the IB Business Services office in 3040 VLSB.
